The risks of inhaled corticosteroid withdrawal in COPD treatment

Most participants in ETHOS and IMPACT already used inhaled corticosteroids, so the dual-therapy comparator withdrew established treatment. Reanalysis suggests the mortality difference may reflect withdrawal harm rather than proof that starting triple therapy reduces deaths.

Recurrent asthma or COPD exacerbations should not be reduced to a single biomarker. Blood eosinophils and FeNO need interpretation alongside symptoms, exacerbation subtype, smoking status, inhaled corticosteroid exposure, treatment changes and adherence uncertainty.
