Summary

Today’s acute-care material begins with anaphylaxis recognition and treatment. Rapidly evolving symptoms across the skin, airway, breathing, circulation or gastrointestinal systems should trigger a clinical diagnosis when the pattern and exposure fit. Skin signs support the diagnosis but are not essential: acute hypotension, bronchospasm or laryngeal symptoms after a known trigger can still represent anaphylaxis. Intramuscular adrenaline into the anterolateral thigh remains the primary treatment, while oxygen, fluids, bronchodilators and antihistamines are adjuncts rather than substitutes.

Persistent symptoms require repeated ABC assessment and further intramuscular adrenaline according to clinical response. Beta-blocker use matters because it may reduce the response to adrenaline; glucagon may be considered because its cardiac effects do not depend on beta receptors. Serum tryptase can support later investigation but must not delay treatment. Recovery also does not complete the pathway: observation for biphasic symptoms, allergy referral, emergency adrenaline access, device teaching and a written action plan all belong in discharge planning.

The paediatric episode focuses on Perthes disease, an idiopathic avascular necrosis of the capital femoral epiphysis. A child aged 4–10 years may present with gradual hip or groin pain, limp, restricted hip movement or apparently isolated knee pain. The hip therefore needs examination whenever a school-aged child has unexplained atraumatic knee symptoms. Plain X-ray is the initial investigation, but normal early films do not exclude the condition when the clinical pattern remains convincing. MRI and urgent orthopaedic input are appropriate when suspicion persists.

The cardiology service announcement adds an educational safety thread ahead of clinical changeover. Starting a new role can increase cognitive load, making local orientation, referral-pathway knowledge and deliberate rest part of safe professional practice. Exam-style questions are most useful when they rehearse reasoning that transfers to ward, clinic and acute cardiology decisions. When anaphylaxis is suspected, use an ABC approach and give intramuscular adrenaline promptly rather than waiting for stridor, hypoxia or cardiovascular collapse. Record the trigger, symptoms, treatment doses, timing and response, then complete observation and discharge planning. In a child with gradual limp or unexplained knee pain, examine hip movement and escalate persistent suspicion even when early X-rays are normal.
