Match the test to the question

Define the clinical question before choosing the test. In chronic coronary disease, coronary CT angiography shows plaque, stenosis and anatomy. Stress echocardiography, single-photon emission computed tomography (SPECT), positron emission tomography (PET) and stress cardiac magnetic resonance address inducible ischaemia, perfusion and ventricular function. Pre-test likelihood, exercise capacity, baseline electrocardiogram, renal function, body habitus, rhythm, prior intervention and local expertise all influence which result will be useful.

An apparently reassuring anatomical study does not always resolve the problem. Persistent typical symptoms with non-obstructive coronaries may reflect ischaemia with non-obstructive coronary arteries, vasospasm or coronary microvascular dysfunction. The next assessment should remain mechanism-focused rather than simply repeating a test that has already failed to answer the clinical question.

Interpret uncertainty in context

Genetic testing creates a related but distinct interpretive problem. A negative test means that no causative variant has been identified with current knowledge and methods; it does not prove that a disorder is non-genetic. A variant of uncertain significance is not a confirmed diagnosis and should not drive irreversible management. Phenotype, family history, patient goals and the limits of the chosen test still matter. Panels, exome or genome sequencing, chromosomal arrays and karyotyping answer different questions.

At the bedside, peripheral neuropathy should first be described by symmetry, distribution and dominant deficit. A symmetrical, length-dependent, sensory-predominant pattern supports a clinical diagnosis, while rapid progression, asymmetry or prominent motor or autonomic features should prompt reconsideration and further testing. Adult attention problems also require a developmental timeline. Childhood examples, collateral history and key transition points matter because bipolar disorder, sleep problems, substance exposure, brain injury and cognition-impairing medicines can mimic or compound attention-deficit/hyperactivity disorder. Checklists and cognitive tests do not establish the diagnosis on their own.

Oral treatment requires selection

In carefully selected streptococcal, staphylococcal and enterococcal infective endocarditis, oral step-down therapy may replace prolonged intravenous treatment. Route alone does not determine efficacy, but safe switching depends on organism susceptibility, clinical stability, source control, oral absorption, regimen feasibility and multidisciplinary agreement. An undrained abscess or incomplete infection control is a reason not to switch. Avoiding long-term venous access may reduce thrombosis, line infection and treatment burden. Oral step-down should not be extrapolated automatically to other Gram-negative or fungal endocarditis.

Before oral step-down for infective endocarditis, confirm susceptibility, clinical stability, source control, oral absorption, regimen feasibility and multidisciplinary agreement.
