An antiviral can have a plausible mechanism, appear in current guidance and still fail to improve survival. That is the problem raised by a recent preprint randomised trial of oseltamivir in patients with laboratory-confirmed influenza who were already critically ill.
The population matters. Participants were aged 12 years or older and required high-flow nasal oxygen, non-invasive or invasive ventilation, or vasopressor or inotrope support. This was not the uncomplicated influenza population from which the average 17-hour symptom reduction is drawn. In that setting, oseltamivir also increases nausea and vomiting. Neither finding answers the critical-care question.
The primary outcome was all-cause mortality at 90 days. Mortality was 14% with no antiviral, 20% after five days of oseltamivir and 19% after ten days. Neither course produced a survival benefit, and the Bayesian analysis generated high probabilities of harm and futility. That is enough to challenge automatic prescribing.
It is not enough to claim a precise causal increase in mortality. The trial was open label and part of an adaptive platform. Many sites did not offer the no-antiviral arm; the oseltamivir groups included patients with greater baseline illness severity; and important co-interventions after randomisation were not collected. Those limitations make the magnitude of any harmful effect uncertain.
Drug exposure was also untidy. Forty-two per cent of participants had received one oseltamivir dose before randomisation, 9% of controls received at least one dose afterwards, and treatment sometimes stopped at discharge. The intention-to-treat result therefore estimates assignment to an oseltamivir strategy rather than a clean comparison between complete treatment and no exposure. That limits what can be said about the drug’s direct effect, but it does not turn the absence of benefit into evidence of benefit.
The bedside problem is the conflict between emerging randomised evidence and guidance that still recommends oseltamivir for severe influenza. A preprint should not trigger an isolated, undocumented change by one clinician. It should interrupt the reflex prescription and prompt an explicit discussion with critical care, infectious diseases, pharmacy and the patient or substitute decision-maker.
For a patient with confirmed influenza who needs organ support, supportive care and investigation for bacterial co-infection remain central. The antiviral decision should be deliberate, multidisciplinary and recorded, with the mortality signal and the trial’s limitations both stated plainly.
