Early extended ECG monitoring finds important rhythms sooner, despite no reduction in recurrent syncope.
A neutral recurrent-syncope outcome does not negate a clear increase in earlier clinically significant arrhythmia detection.
A test can find substantially more disease without improving the outcome chosen to judge whether the strategy works. That is the interesting tension in a trial of early extended ECG monitoring after unexplained syncope.
The pragmatic study randomised 2,234 adults across 45 UK emergency departments to standard care or a 14-day ambulatory ECG monitor fitted soon after presentation. Its primary outcome was patient-reported recurrent syncope at one year. There was no significant difference between the groups.
Look only at that result and the intervention appears disappointing. Look at what happened diagnostically and the picture changes.
Clinically significant rhythms were detected in 22% of patients receiving early monitoring compared with 9% receiving standard care. Diagnosis was also faster, occurring at a mean of 22 days rather than almost 55 days. Earlier rhythm detection led to more rhythm-directed treatments, including pacemakers, anti-arrhythmic drugs and implantable defibrillators.
There is no contradiction here. Syncope is a presenting complaint with several possible mechanisms. In the monitoring group, 78% had no cardiac rhythm disturbance detected. A device designed to identify intermittent arrhythmias cannot be expected to prevent recurrent episodes caused by something else.
The mortality result is more intriguing but needs considerably more restraint. One-year mortality was 1.5% with early monitoring and 2.9% with standard care. This was an unexpected secondary outcome. The increase in rhythm-directed treatment gives the finding biological plausibility, but it does not establish why mortality differed or prove that monitoring caused the difference.
For emergency services considering extended monitoring, the implementation problem matters as much as the device. Fitting monitors ad hoc in a crowded department and leaving report ownership uncertain creates another opportunity for failure. A trained ambulatory or same-day service can provide prompt fitting, receive reports and arrange onward treatment or referral.
The practical implication is to judge early monitoring for what it can actually do. It may not stop another faint, but in selected patients with unexplained syncope it can identify important intermittent arrhythmias earlier. That benefit only reaches the patient if someone is clearly responsible for acting on the result.
