Confirming papilloedema

Buried optic disc drusen, crowded or tilted discs, myelinated nerve fibres and other structural variants can all produce an elevated appearance.
The assessment needs more than a quick look through an undilated pupil. Record acuity, colour vision, pupils and cranial nerves, then obtain dilated fundoscopy and formal visual fields. A markedly enlarged blind spot, nasal step or peripheral constriction is clinically important even when acuity remains good. A relative afferent pupillary defect is unusual in symmetrical raised intracranial pressure and should raise concern about severe asymmetric optic nerve dysfunction or another diagnosis.
Optical coherence tomography (OCT) helps grade and monitor swelling, but the automated retinal nerve fibre layer number is not enough. Segmentation errors can distort the result. Review the volume scan and macular images for fluid within or beneath the nerve, hyperreflective spots and fluid tracking towards the macula.
Protecting vision while finding the cause
Symptoms alone do not prove raised intracranial pressure, but their trajectory helps establish urgency. Transient visual obscurations at rest and increasing pulse-synchronous tinnitus are warning signs. Baseline documentation matters because deterioration may be seen in the fields before it appears on the acuity chart.
The initial work-up should include blood pressure, glucose, full blood count and urea and electrolytes. Review medicines, supplements and symptoms of sleep apnoea. Brain imaging must assess for a space-occupying lesion and include venography to exclude cerebral venous sinus thrombosis. Postponing venous imaging leaves an important secondary cause unresolved.
Formal fields and disc severity decide when investigation becomes an emergency.
