Positive FND diagnosis and precise assessment of swollen optic discs

Papilloedema is not a synonym for swollen optic discs; it specifically means optic disc swelling caused by raised intracranial pressure.

Summary

Two neurological diagnostic themes frame today’s briefing. Functional neurological disorder should be diagnosed using positive clinical features rather than treated as the label left after every investigation is normal. Naming the condition clearly and explaining that symptoms are real, brain-based and clinically recognised can reduce stigma. Shared language across neurology, psychology, physiotherapy, occupational therapy and speech therapy also helps prevent fragmented or contradictory explanations.

Swollen optic discs require equally precise terminology. Papilloedema specifically means optic disc swelling caused by raised intracranial pressure; optic nerve drusen, tilted discs, myelinated nerve fibres and optic neuropathies can produce alternative appearances. Fundus examination, optical coherence tomography and formal visual fields provide complementary information. Spontaneous venous pulsation supports normal intracranial pressure when present, but its absence is not diagnostic. In pulse-synchronous tinnitus, a change with gentle jugular compression supports a venous-flow mechanism.

The wider practical neurology material reinforces active enquiry during follow-up. Parkinson’s care should include sleep, mood, constipation, orthostatic symptoms, exercise, diet, driving and social function rather than focusing only on tremor or levodopa dose. Bladder, bowel and sexual dysfunction are frequently under-reported and need direct, neutral questioning. In neurocritical care, a normal sub-hairline EEG should not override ongoing clinical concern because limited scalp coverage can miss posterior or parasagittal seizure activity.

Iron deficiency provides the haematology and physiology thread. Reactive thrombocytosis appears to reflect a targeted shift within megakaryocytic-erythroid progenitors towards platelet production, together with larger and more polyploid megakaryocytes. The finding should still be interpreted clinically: iron studies, bleeding history, inflammation and other reactive causes matter. Correcting the iron deficiency addresses both the anaemia and the stimulus for thrombocytosis.

The lupus episode adds a trial-appraisal problem. Dapirolizumab pegol blocks CD40 ligand signalling, affecting B-cell, T-cell and interferon-related pathways. Although PHOENYCS-GO met its week 48 BICLA primary endpoint, failure of the earlier week 24 endpoint within the statistical hierarchy limits formal claims about later outcomes. Composite responses do not reveal which symptoms improve for an individual patient, and treatment decisions must also consider hypersensitivity, thromboembolic risk, infection, infusion logistics and the absence of clear patient-reported outcomes.

When optic discs appear swollen, document the visual symptoms and examination findings before applying the label of papilloedema, and use optical coherence tomography or formal visual fields where appropriate. In unexplained neurological presentations, look actively for positive features supporting FND and explain the diagnosis using direct, validating language. When thrombocytosis accompanies iron deficiency, review iron studies and possible bleeding rather than treating the platelet count as an isolated abnormality.
