Rethinking interruptions in the emergency department

A safer ED communication system separates urgent interruption from messages that can wait.

Summary

Interruptions in an emergency department are usually framed as an irritation. The more useful question is whether they are consuming attention that should be protecting patients.

One local ED has redesigned ECG review around that distinction. Rather than interrupting a doctor for every tracing, non-urgent ECGs go to a marked review area and still require physician interpretation within ten minutes of acquisition. Immediate interruption remains the rule when the machine flags ST-elevation myocardial infarction (STEMI), the patient looks unwell, vital signs are unstable or a member of staff is concerned.

That last condition matters. A workflow intended to protect attention becomes unsafe if it also raises the threshold for speaking up. Machine reassurance cannot overrule clinical concern, and a quick “no STEMI” glance is not a substitute for full ECG interpretation. The episode describes a third-degree heart block being overlooked while the clinician was dictating.

The local audit is interesting but should not be oversold. Among more than 7,000 ECGs there were 15 infarction cases. The machine flagged 13, while clinicians identified two unflagged occlusion infarctions. More than 3,500 interruptions per unflagged case is a striking workload comparison, but it is not evidence that deferred review improves patient outcomes. The two unflagged occlusion infarctions also did not meet traditional STEMI criteria on retrospective review.

The broader design principle is to separate urgent communication from communication that can safely wait. Non-urgent secure messages can be reviewed between tasks, while urgent concerns retain immediate access. That protects nursing attention as well as medical attention and avoids turning interruption reduction into a request for doctors simply to be left alone.

Changing communication systems creates another risk: assuming silence means agreement. It may instead reflect confusion, resignation, fear, fatigue or a belief that speaking up will achieve nothing. A practical “true echo” check asks colleagues what they heard, how the change affects their work and what may be missing. Workarounds and quiet withdrawal should prompt curiosity rather than dismissal.

So the goal is not an interruption-free department. It is a department where urgency determines the route of communication, where deferred tasks have a reliable review system, and where staff concern can still cut straight through it.

Notice which interruptions truly require immediate attention and which can wait until the current task ends. If adapting deferred ECG review or non-urgent messaging, make the urgent override criteria explicit and confirm that everyone understands them. Treat silence after a workflow change as ambiguous and check how the change is actually landing.

A patient taking rivaroxaban presents after severe vomiting with black vomit. Haemoglobin is initially 135 g/L and the patient remains haemodynamically stable. Does the normal initial haemoglobin exclude recent upper gastrointestinal bleeding?

No. An initially normal haemoglobin does not rule out a recent bleed, so symptoms, further vomiting or melaena, haemoglobin and renal function should be reassessed as the clinical picture evolves.
