A ventricular rate above 100 beats/min can meet the definition of atrial fibrillation with rapid ventricular response, but the number alone does not tell you what to treat first. The useful distinction is whether the arrhythmia is driving the patient’s deterioration or whether another illness is driving the rate.
In established atrial fibrillation, sympathetic stress can increase atrioventricular nodal conduction. Infection, hypoxia, dehydration, electrolyte disturbance, thyrotoxicosis, pulmonary embolism, alcohol and missed medication are all potential drivers. In a haemodynamically stable patient with a modest rate rise, the sensible approach is to treat the precipitating problem and reassess before adding intravenous rate control. It is also worth checking whether regular rate-control medication has simply been missed.
The decision changes when atrial fibrillation is causing shock, ongoing hypotension, ischaemia, altered consciousness or acute heart failure. That is a rhythm problem with immediate physiological consequences. Urgent synchronised cardioversion is indicated, using the defibrillator’s highest recommended energy and considering an anteroposterior pad position.
There is another ECG pattern that deserves particular caution. A very rapid irregular rhythm around or above 200 beats/min, especially with QRS complexes varying in width and morphology, raises concern for pre-excited atrial fibrillation. In that setting, atrioventricular nodal blocking drugs such as adenosine, verapamil or a beta blocker should be avoided.
The same discipline applies after the rate settles. A first diagnosis of atrial fibrillation still needs prompt assessment of stroke and bleeding risk, even if symptoms have resolved or the rhythm is already rate controlled.
The practical job is broader than lowering a heart-rate number. Confirm the rhythm, identify adverse features, establish onset, look for reversible drivers and recognise pre-excitation before choosing rate control, rhythm control or treatment of the precipitant. Getting that sequence wrong risks treating a compensatory response while missing the illness or conduction pathway that actually needs attention.
