When liver tests mislead in cirrhosis

Modest transaminases can mask worsening function, while INR alone cannot define haemostatic balance in cirrhosis.

Do not give plasma simply to normalise an elevated INR in cirrhosis; haemostasis may favour either bleeding or thrombosis.

Summary

A patient with cirrhosis can deteriorate while the aminotransferases remain remarkably unimpressive. That is an easy laboratory pattern to misread.

Aspartate aminotransferase and alanine aminotransferase mainly reflect hepatocellular injury. Bilirubin and INR are more useful markers of liver function. Rising bilirubin and INR can therefore represent worsening function even when AST and ALT provide little visual drama on the blood results.

INR creates a second trap. Cirrhosis reduces production of both procoagulant proteins and natural anticoagulants. The result is a fragile, rebalanced haemostatic state that can move towards either bleeding or thrombosis. An INR of 2 in cirrhosis therefore cannot be interpreted as though it represents the same haemostatic state as an INR of 2 during warfarin treatment.

That has practical consequences. Plasma should not be given simply to normalise an elevated INR. When bleeding is present or an invasive procedure is planned, the clinical situation and available viscoelastic testing can help target blood products. An elevated INR alone is also not a reason to withhold pharmacological venous thromboembolism prophylaxis.

Volume assessment can be equally deceptive. Marked ascites and apparent fluid overload can coexist with intravascular depletion. Fluid treatment therefore needs repeated reassessment rather than successive boluses aimed simply at improving urine output. Albumin has defined roles, including spontaneous bacterial peritonitis and large-volume paracentesis, but it is not the default resuscitation fluid for every hypotensive patient with cirrhosis.

Hepatic encephalopathy brings another tempting number: ammonia. The diagnosis is primarily clinical, and the absolute ammonia concentration correlates poorly with severity. Altered mental status still requires a search for precipitants and alternative causes, including infection, gastrointestinal bleeding, hypoglycaemia, hypercapnia, medication effects, stroke, seizure and electrolyte disturbance.

When lactulose is used, the bedside target is two or three soft bowel movements each day rather than normalisation of the ammonia result. Excessive treatment simply replaces one problem with another.

The larger lesson from decompensated liver disease is practical rather than biochemical. Use the laboratory results that answer the clinical question, reassess physiology repeatedly and watch the trajectory of organ failure. In acute liver failure or severe acute-on-chronic liver failure, early transplant-centre involvement should occur before every other option has been exhausted.

What to change on your next shift

In deteriorating cirrhosis, separate markers of hepatocellular injury from markers of liver function rather than relying on AST and ALT alone. Avoid plasma simply to correct the INR, and use the bleeding or procedural context with available viscoelastic testing. In suspected hepatic encephalopathy, assess clinically and titrate lactulose to two or three soft bowel movements rather than a target ammonia concentration.
